"Oregon is about to redesign the Certificate of Immunization Status (CIS) used in schools and children's facilities in Oregon. This is the document that parents or physicians complete to document a child's immunization history for school and child care enrollment. Since we need to rethink how the information is organized, we would like to see examples of forms from other states."
